The aim of this programme of work is to produce a pathway and additional information that will support all commissioners and providers to deliver improved care to the people in the South West; care which will also meet their individual needs and use resources wisely, as the population gets older with more complex needs. The SCN has written to all commissioners to share this work.
The SCN will continue to champion the pathway and support organisations to embed this way of working in the South West.
