The Long Term Conditions working group has now been disbanded.
The Long Term Conditions Group brings acute trust clinicians and managers together with commissioners and Public Health England to map current services across the South West.
The group meets bi-monthly to identify potential variations, service gaps and areas of good practice with the aim of developing commissioning recommendations for service provision.
Currently the group is supporting the implementation of the healthcare aspects of the new SEN reform legislation due in September 2014.
