The Transition working group has now been disbanded.
This group of clinicians and managers of acute trusts will meet with CCG commissioners, NHS England and Public Health England representatives to discuss child to adult transition services across the South West.
An analysis of current transition practice within the South West has been completed and links formed with the national NHS Improving Quality (IQ) team and their work on transition. This network group will meet bi-monthly to develop a framework for transition in line with the work of NHS IQ.
