Moving Transition Forward in the South West

Moving Transition Forward in the South West, on 23rd September 2014 brought transition teams from 13 provider trusts together with commissioners from 9 CCGs and two area teams, and patient groups.

The Maternity & Children’s SCN has worked with all 14 provider trusts in the South West to populate a dashboard built on the principles of good transition identified in the national Generic Service Specification for Transition. This dashboard gives, for the first time, a clear picture of the current provision on offer to children and young people transitioning from paediatric to adult care across the region.

Reflecting on good practice and areas needing improvement highlighted by the data, teams formulated action plans to take back to their trusts with a view to moving forward targeted improvement in transition services.

The next steps for the SCN include roll out of a patient and parent experience survey to acute trusts and support to commissioners looking to improve transition services via their contracting arrangements.
